Chronic persistent headache.
Patient Data
The visualized bony outlines show widened diploic spaces and a ground glass matrix with intact cortical outlines. No periosteal reaction. Absent pneumatization and obliteration of the paranasal sinuses consistent with total aplasia of the paranasal sinuses. Unremarkable average dimensions of the optic canals, superior and inferior orbital fissures, as well as the infraorbital foramina on both sides with no CT features of osseous compression.
Case Discussion
The paranasal sinuses are subject to marked variation among individuals and between sides in the same person, as regard (aeration) and bony septae. Isolated frontal sinus agenesis is a common radiological finding but total paranasal sinus agenesis, as in this case, is very rare.
