Partial tear of inferior gemellus muscle

Case contributed by Dr Utkarsh Kabra

Presentation

Left hip pain.

Patient Data

Age: 40 years
Gender: Female

Hyperintensity in left inferior gemellus muscle on STIR and T2 fat sat images with partial discontinuity of fibers is seen.

Subtle hyperintensity is also seen in left quadratus femoris muscle.

Note is made of possible thickening of posterior junctional zone of uterus, nabothian cysts in cervix and cystic hemorrhagic lesion in bilateral adenexa.

Case Discussion

Hyperintensity in left inferior gemellus muscle on STIR and T2 fat sat images with partial discontinuity of fibers, consistent with partial tear (grade II strain).

Subtle hyperintensity is also seen in left quadratus femoris muscle consistent with grade I strain.

The differential diagnosis, in this case, would be of ischiofemoral impingement. However, no obvious narrowing of ischiofemoral space is seen, essentially ruling it out.
