Patient has had complete laryngectomy for squamous cell cancer of larynx. There is a Passy-Muir valve covering the tracheostomy.

Look at old images if available.
Look at all foreign bodies in detail.
When you look at neck and trachea the air overlying the larynx is wider than usual, this should clue you into it.
