Presentation

Anterior knee pain. Patient runner of long distances. No trauma.

Patient Data

Age: 30 years
Gender: Female

Hoffa fat edema in appearance superolateral.

Minimum joint effusion.

Diffuse swelling and irregularities of the patellar cartilage surface and deep fissure in the medial facet (chondropathy), with the subchondral bone preserved.

Hoffa fat edema in appearance superolateral, in exacerbated request function hyperextension of the extensor mechanism.

Case Discussion

This condition results from the overstimulation of the knee extensor mechanism due to friction during physical activities and results in edema of Hoffa's fat in the inferior aspect of the patella. It is often related to patella alta.

In this case, the patient has been exercising daily with running activities and recently presented with anterior knee pain. Within this medical history and MRI findings, the diagnosis of Hoffa fat pad impingement syndrome should be considered.
