Perforated gastric ulcer with pneumoperitonum

Diagnosis almost certain

Presentation

Sudden onset of severe epigastric pain.

Patient Data

Age: 50 years
Gender: Female

The CT scan images demonstrate:

  • a focal defect of the anterior gastric wall at the greater curvature with adjacent perigastric fat stranding as well as fluid collection
  • a large amount of free gas within the peritoneal cavity mainly around the liver, spleen and within the umbilical hernia with moderate thickening and enhancement of the peritoneal reflections
  • small adrenal nodules (incidentalomas), one on the right and two on the left (the mean density on non-contrast CT was 2 HU), probably adenomas
