Pigmented villonodular synovitis - knee

Diagnosis probable

Presentation

Locking of knee while walking.

Patient Data

Age: 30 years
Gender: Male

A well defined nodular lesion is seen within knee joint appearing heterogenously hypointense on T1W and T2W images. It is seen to indent Hoffa's fat pad, and seen close to tibial attachment of ACL.

Moderate knee joint effusion.

Case Discussion

The possibility of pigmented villonodular synovitis (PVNS) is considered. Differential diagnoses include 'cyclops lesion', and intra-articular chronic hematoma. However, in view of the lack of trauma-related history, and intact ligaments, PVNS merits primary consideration.

The patient is not ready for any intervention and sustaining similar symptoms since the study done previously. Pathological diagnosis is unavailable.
