Referred for further evaluation of an unusual suprasellar tumor.

Mass centered on the third ventricle, occluding the foramen of Monro and reaching down to the sella turcica. The mass has cystic and intensively enhancing vascular components.
Hydrocephalus of the lateral ventricles. VP shunt via left parieto-temporal approach, crosses through the left ventricular trigone, its tip apparently in the left ambient cistern.

Mass with cystic areas fills and dilates the third ventricle including the foramen of Monro, insinuates itself around the terminal portions of the ICAs and basilar artery, fills and widens the sella turcica, and fills the interpeduncular and prepontine cisterns. The solid portion of the mass enhances intensively post-contrast.

The lateral ventricles are dilated, VP shunt placed as described on CT study.

Diagnosis:

BRAIN, SELLAR/SUPRASELLAR TUMOR, RESECTION:

PILOCYTIC ASTROCYTOMA, WHO GRADE I (SEE COMMENT).

Comment:

Although myxoid change is prominent in specimen 1, the other feature of pilomyxoid astrocytoma, i.e. perivascular cellular arrangement is lacking. Also, focal biphasic pattern, perivascular hyalinization and eosinophilic granular bodies are observed.
