Plantar fasciitis

Diagnosis almost certain

Presentation

Complaining of pain in the sole of midfoot for the last two months. No history of trauma. Referred for ultrasound evaluation of the tendon region.

Patient Data

Age: 45 years
Gender: Female
ultrasound

A short axis and multiple long axis images of the plantar fascia on the symptomatic show abnormally fascia in the midfoot. It is hypoechoic, thickened, with a perifascial fluid cleft. Local hypervascularity is present. Proximal fascia shows normal echopattern. There is no spur formation.

The asymptomatic side was examined for comparison and it shows normal fascia.

Case Discussion

Findings of plantar fasciitis affecting predominantly in midfoot. Pathology correlates well with the site of pain pointed by the patient. There is no fibroma formation in this case.

Plantar fasciitis commonly involves proximal third fibers - near calcaneum insertion.

Two other distribution of pathology. One shows proximal involvement extending to the mid-third foot. The other is isolated mid-third-foot involvement.
