There is a large, lobulated mass centered on the left para pharyngeal space, arising from the deep lobe of the left parotid gland.  The lesion is isointense to muscle on T1, predominantly hyperintense on T2, and shows enhancement. There is moderate narrowing of the airway caused by medial displacement of the pharyngeal mucosal space.

MICROSCOPIC DESCRIPTION:
Sections show a tumor, composed of variably cellular areas admixed with areas within which there is a myxoid connective tissue stroma, within which scant myoepithelial cells are seen. Within the glandular areas, glands are seen, some with tubular arrays of uniform cells with a fine nuclear chromatin
pattern and ovoid nucleus with a small amount of surrounding cell cytoplasm.
The tumor is present throughout the tissues and extends to the resection margins. No vascular or perineural invasion is seen.

DIAGNOSIS/COMMENT:
Deep parotid left/parapharyngeal tumor mass - pleomorphic adenoma, present at resection margins.
