Pleomorphic adenoma - parotid

Presentation

Painless swelling of the right parotid region.

Patient Data

Age: 50 years
Gender: Male
mri

Well-encapsulated lobulated right parotid mass on its lower part with deep lobe extension. It shows intermediate to low signal on T1, heterogeneous high signal on T2 with a peripheral hypointense rim (capsule) and no restricted diffusion. The postcontrast dynamic sequences show a progressive and gradual enhancement of the tumor.

Normal appearance of the left parotid gland.

No cervical lymphadenopathy.

Case Discussion

MR features are suggestive of pleomorphic adenomas of the right parotid gland.

The patient underwent complete resection of the tumor with histopathological exam that confirmed the diagnosis of pleomorphic adenoma with a capsular pseudoinvasion that was attributed to the fine-needle aspiration (FNA) performed before surgery, however, a follow-up was recommended.
