Posterior inferior cerebellar artery (PICA) infarct

Diagnosis almost certain

Presentation

Vertigo and ataxia.

Patient Data

Age: 60 years
Gender: Male

Multiple areas of low signal on T1, high signal on T2 and FLAIR with restricted diffusion are seen in the posteroinferior and inferomefial aspect of the left cerebellar hemisphere in keeping with PICA territory infarct. 

 Parenchymal volume loss mainly at the frontal regions with widening of the cortical sulci and moderate ventriculomegaly. Small vessel ischemic change in both the cerebral hemispheres.

Case Discussion

MRI features of multiple areas of restricted diffusion in the posterior inferior cerebellar artery territory consistent with PICA infarct.

 MRI is far superior to CT in the sensitivity of posterior circulation strokes.
