Primary splenic lymphoma (marginal zone lymphoma)

Presentation

Upper left abdominal pain. Splenomegaly first detected on ultrasound.

Age: 50 years

Splenic diameter about 20 cm in the long axis. Subhepatic enlarged lymph nodes.

Massive enlargement of the spleen with slightly inhomogeneous parenchymal enhancement. Multiple enlarged lymph nodes around the celiac trunk, the hepatoduodenal ligament, and the inferior vena cava. Thoracic and pelvic nodes were not involved (not shown).

Case Discussion

Splenic marginal zone lymphoma is a rare, indolent subtype of non-Hodgkin lymphoma, which accounts for less than 2%. Dull abdominal pain from splenomegaly is often the only symptom 1.
