Probable combined pulmonary fibrosis and emphysema (CPFE)

Case contributed by Roberto Schubert
Diagnosis probable

Presentation

Outpatient with lung fibrosis, allegedly UIP. Deteriorating blood gases.

Patient Data

Age: 70
Gender: Male

Case Discussion

Though there are interstitial reticular opacities, some ground-glass opacities in the posterior lingula, and subpleural fibrosis, the pattern of translucencies is more typical for paraseptal emphysema than honeycombing. The UIP-typical apico-basal gradient is lacking, as well.
