Right flank pain. Recent ureteral colic on the right.

Nephrographic-excretory images shows multiple filling defect with smooth margins and regular shape in all calyxs bilaterally.

Note the presence of a few minute calcific stones in the right kidney and a tiny left renal cortical cyst.

Case Discussion

Prominent renal papilla are a benign anatomical variant, simulating a filling defect in the excretory phase. The conservation of the shape of the fornix, the smooth contours and the frequent presence of multiple similar aspects differentiate it from a neoplasm.
