Focal suspicious lesion at the anterior transition zone (mid gland to base).
On T2W, the lesion is hypointense, with a lentiform shape and ill-defined margins. It shows the "erased charcoal sign".
On DWI, there is restricted diffusion with a low ADC value of 600.
On the dynamic contrast enhanced scans, the lesion demonstrated a Type 3 curve.
The overall PI-RADS score is 5.
Case Discussion
A targeted biopsy of the left anterior transition zone (TZ) lesion, confirmed a Gleason 4+3 = 7, carcinoma of the prostate gland.
