Presentation
History of antibiotic use, diarrhea, leukocytosis and abdominal pain.
Patient Data
There is evidence of thickened colon wall involving the ascending, descending and transverse colon (accordion sign). Minimal pericolic fatty stranding, multiple enlarged mesenteric lymph nodes, and minimal pelvic free fluid. The superior and inferior mesenteric artery are well opacified.
The annotated images illustrate the accordion sign, pericolonic lymph nodes and pelvic free fluid.
Case Discussion
Pseudomembranous colitis is an infectious colitis caused by Clostridioides difficile producing toxins. There is usually a history of long antibiotic use associated with abdominal pain and diarrhea. It is important to recognize the radiological imaging features to prevent complications such as perforation. The diagnosis was confirmed by stool samples which reveal C. difficile.
