Rapidly growing small bowel lymphoma

Diagnosis probable

Presentation

Young man with abdominal pain and weight loss. Presented 4 days later with vomiting. Normal laparoscopy 3 months ago.

Patient Data

Age: 20
Gender: Male

CT on second presentation

Large right iliac fossa mass intimately related to and obstructing the distal ileum (arrow) with extrinsic compression of the cecum and ascending colon (arrow).  Note ascites and nodularity in the peritoneum, suggestive of peritoneal infiltration.

CT performed 4 days previously

Scan 4 days prior showed the same mass albeit a little smaller and not obstructing the small bowel!

Case Discussion

There are very few tumors that grow at such a rapid pace - lymphoma is the most likely.  This growth means that cell turnover is rapid, thus medical treatment is often very effective.
