Recent anterior dislocation of the right shoulder and posterior dislocation of the left shoulder

Presentation

Recent violent trauma.

Patient Data

Age: 29
Gender: Male

Evidence of recent anterior dislocation of the right shoulder and subsequent:

  • Coracoid process avulsion fracture with resultant rotator interval tear and mild subscapularis tendinopathy.
  • Hill-Sach’s lesion of the right humeral head.
  • Moderate joint effusion and subcoracoid bursitis.

Joint is now relocated. 

Evidence of recent posterior dislocation of the left shoulder and subsequent:

  • Trough lesion (reverse Hill-Sachs lesion) of the left humeral head.
  • Low grade strain of the left infraspinatus, teres minor, posterior fibers of supraspinatus and posterior fibers of deltoid muscles.
  • Moderate joint effusion and subcoracoid bursitis.

Joint is now relocated.

Case Discussion

This case shows the uncommon incidence of concurrent anterior dislocation of the right shoulder (with subsequent Hill-Sach’s lesion and coracoid process avulsion fracture) and posterior dislocation of the left shoulder (with subsequent Trough lesion and strain of the left infraspinatus, teres minor, posterior fibers of supraspinatus and posterior fibers of deltoid muscles).
