Diagnosis almost certain

Presentation

24-25 week premature infant (750-1000 g). Increasing oxygen requirements (FiO2 42-48%).

Patient Data

Age: 3 weeks old
x-ray
  • lung volumes are mildly decreased
  • diffuse hazy lung opacification with diffuse bilateral granular opacities
  • orogastric tube, with tip in the stomach

Case Discussion

Respiratory distress syndrome (RDS) is a consequence of surfactant deficiency in premature infants. It can worsen and improve relatively rapidly. In this case, the diffuse hazy opacification of the lungs indicates relatively severe RDS.
