Retrofenestral otosclerosis

Presentation

Hearing loss; conductive deafness on audiometry in both ears; no discharge.

Patient Data

Age: 35 years
Gender: Male

A small lucency/cleft is seen adjacent to the oval window region bilaterally in the fissula ante fenestram. This is associated with patchy lucency around the cochlea, also bilaterally, most easily seen around the basal turn on the right. 

On the left there is evidence of prior stapes implant. 

Case Discussion

This case illustrates bilateral fenestral and retrofenestral otosclerosis. Cochlear involvement can be considered the more advanced disease, nearly always also having fenestral involvement too. it associated with both sensorineural and conductive hearing loss.
