Rheumatoid arthritis: wrist MRI

Diagnosis almost certain

Presentation

Pain and swelling at the wrist since 6 months ago.

Patient Data

Age: 35 years
Gender: Female

MRI images demonstrate extensive synovitis about the carpal bones especially in the radiocarpal, intercarpal, and carpometacarpal joints associated with carpal bone marrow edematous changes. Bone marrow edematous changes from 2nd  to 4th metacarpal bases also is seen.

Case Discussion

Synovitis is one of the first abnormalities seen in rheumatoid arthritis. Joint cavity, tendinous sheath and bursa are lined by the thin synovial tissue that is not visible normally on MR images.

Increased volume and thickening of synovial tissue associated with increased water content and contrast enhancement on MR imaging identified as synovitis caused by rheumatoid arthritis1,2,3
