There is severe cardiomegaly, with mild changes of pulmonary edema. No focal airspace consolidation or pleural effusion. Right chest wall dual-lead pacemaker is in situ.

Severe cardiomegaly with an especially large right atrium. Evidence of right heart failure with reflux of contrast into the hepatic veins. Dual lead pacing device.
Case Discussion
This patient has a known history of tricuspid regurgitation.
In cases of severe cardiomegaly, signs on chest x-ray become non-specific.
