Right iliac wing fracture

Case contributed by Andrew Murphy
Diagnosis certain

Presentation

Fall onto right hip from pushbike.

Patient Data

Age: 70 years
x-ray

There are two linear lucencies at the periphery of the right iliac wing, oblique and vertical in orientation. Both these lucencies break the cortex of the iliac crest. These fracture lines my appear to be communicating, resulting in a triangular fragment.

This appearance consistent with a Duverney fracture.

Case Discussion

Duverney fractures occur as a result of direct trauma to the ilium, in the context of this case; a fall from a bicycle onto the right side. Obliques are not routinely performed in the pelvic series however, in this case, the radiographer has performed this additional view to better demonstrate the fracture.
