Right middle lobe bronchiectasis

Presentation

Persistent cough.

Patient Data

Age: 55 years
Gender: Female
x-ray

Frontal chest radiograph shows bilateral reticulations and lucencies at the right paramediastinal mid and lower lung zones, and to a much lesser extent, the left lower lung zone, in keeping with bronchiectasis. 

Non-enhanced axial CT scan of the chest with reconstructed images into sagittal and coronal planes demonstrate cystic bronchiectasis confined to the right middle lobe and to a much lesser extent, the lingula. 

Case Discussion

Lady Windermere syndrome is a recognized cause of bronchiectasis that is confined to the right middle lobe and lingula, with or without centrilobular nodules. The syndrome typically affects middle aged women who tries hard to suppress their coughs. The syndrome is named after 'Lady Windermere's Fan, A Play About a Good Woman' by Oscar Wilde.
