Acute saddle embolus extending to the right and left main pulmonary arteries as well as bilateral lobar and segmental branches.
The pulmonary trunk is prominent measuring 3.7 cm in maximum transverse diameter compared to 3.5 cm in maximum transverse diameter of the ascending aorta at the same level.
Straightening of the inter-ventricular septum indicating right heart strain.
Reflux of contrast in IVC and hepatic veins.
Bilateral basal atelectasis.

Acute saddle pulmonary embolus extending into both lobar and segmental branches with evidence of pulmonary arterial hypertension and right heart strain.
