Physeal fracture of the distal fibula. It is best seen on the oblique view and is displaced. There is a tiny metaphyseal fragment making it a type II Salter-Harris fracture.

Zoomed view of the oblique x-ray highlighting the displaced fibular fracture. There is tiny ossific density from the metaphysis attached to the fracture fragment.
Case Discussion
Salter-Harris fracture of the distal fibula. These are almost always Salter-Harris type II fractures, even if the metaphyseal fragment is tiny.
