Sapheno-femoral junction incompetence

Presentation

Dilated veins along medial aspect of right thigh and leg with pruritus, skin discoloration and recurrent ulceration over right lower leg.

Patient Data

Age: 30 years
Gender: Male
ultrasound

Sapheno-femoral junction is incompetent with reversal of flow on Valsalva maneuver. Great saphenous vein is dilated throughout its course. Multiple dilated subcutaneous veins are seen along medial aspect of lower thigh and leg suggestive of varicose veins. Dilated perforator veins connecting subcutaneous veins with deep venous system were seen at medial aspect of ankle and mid-leg (Cockett's perforators).

Note diffuse thickening of subcutaneous tissue in leg and thigh.

Skin pigmentation and scarring due to healed ulcers over medial aspect of lower right leg consistent with changes of lipodermatosclerosis.
