Scaphoid fracture - distal third

Presentation

Motorcycle accident. Tenderness over the anatomical snuff box.

Patient Data

Age: 25 years
Gender: Male
x-ray

Hand radiographs revealed a fracture through the scaphoid. The fracture line is oblique and goes through the scaphoid waist to its distal third.

This distal third oblique fracture of the scaphoid can be classified as a Herbert type B1.

Case Discussion

Scaphoid fractures are the most common fracture of the carpus. They are usually the result of a fall from standing height. Other mechanisms of injury are more commonplace among a younger population, e.g. sports trauma, direct blow, assault, motor vehicle collision.

It is important to recognize which segment of the scaphoid is fractured, as the location of the fracture is related to the risk of avascular necrosis and patient management options.

In the present case, there is a distal third oblique fracture, a Herbert type B1. Other commonly used classification system is the Mayo classification.
