Sesamoid fracture of left thumb

Presentation

History of acute headache, neck pain and bilateral hand/wrist pain following an MVC.

Patient Data

Age: 20 years
Gender: Female
x-ray

Mildly displaced fracture through the ulnar sesamoid bone of the thumb. Moderate soft tissue swelling around the MCP joint of the thumb.

Case Discussion

A bipartite sesamoid bone is a normal anatomic variation of a sesamoid bone in which the sesamoid bone is in two pieces that is present from birth. Direct trauma to a bipartite sesamoid bone can present very similarly to a fracture of a sesamoid bone and can be difficult to distinguish from each other.

Imaging of a sesamoid fracture compared to a bipartite sesamoid will show irregular margins, possible comminution, and exaggerated separation of the fragments. In contrast, imaging features of a bipartite sesamoid include smooth, well-rounded borders and equally separated fragments 1.
