Presentation

Foot pain over 1st metatarsal head

Patient Data

Age: 12
Gender: Male
X-ray

Medial sesamoid is slightly sclerotic (arrow) and irregular in shape. 

Medial great sesamoid is also bipartite (arrow) as is the lateral.

MRI

Bone marrow edema in the medial sesamoid (arrow in axial). Bipartite medial great sesamoid with bone marrow edema on either side of the syndesmosis.

Minor soft tissue edema overlying the sesmoid (arrow in sagittal).
