Presentation

Lower cranial neuropathy in a 45 year old male with bilateral ear discharge. Non-diabetic, no known immune-compromised state. There was restriction of jaw movements on examination

Age: 45 years
Gender: Male

Extensive enhancement of the skull base, with associated soft tissue, involvement of the right zygomatic arch and the pterygoid muscles on the right.

The temporomandibular joint on the right side is also involved.

There was otitis media on both sides.

Case Discussion

The patient steadily improved with antibiotic therapy.

Final Diagnosis: Skull base osteomyelitis and right temporomandibular joint septic arthritis.
