Small bowel obstruction secondary to femoral hernia

Diagnosis almost certain

Presentation

Abdominal pain and distension.

Patient Data

Age: 95 years
Gender: Male

Small bowel obstruction due to left femoral hernia. No signs of intestinal vascular compromise. Small volume of free fluid in the hernia sac. The left inguinal canal with fluid is seen immediately medial to the hernia. 

Non-obstructed small bowel loops in a right indirect inguinal hernia. 

Hiatus hernia.

Bilateral pleural effusion and septal enlargement secondary to heart failure. 

Other findings; mild ascites, uncomplicated diverticular disease, left renal cyst and multiple hypodense hepatic lesions with peripheral focus of enhancement that may represent hemangiomata.

Case Discussion

The patient went to surgery and the findings of a left femoral hernia with signs of incarceration  were confirmed.
