Spigelian hernia

Diagnosis almost certain

Presentation

Left lower abdominal wall swelling; more prominent during physical exercise - 15 days.

Patient Data

Age: 65 years
Gender: Male
ultrasound

A heterogeneous hyperechoic lesion (24 x 21 x 11 mm) with internal linear strands noted in left lower infraumbilical abdominal wall. The lesion is solid with few blood vessels. It is located between lateral margin of left rectus abdominis and medial border of left oblique muscle.
It extends superficial to left external oblique. Neck of lesion measures 11 x 10 mm.
Lesion is not reducible on compression.
