Spinal cord metastasis

Diagnosis almost certain

Presentation

Known case of colon cancer with brain and lung metastasis post-surgery and radiotherapy presented with lower limb weakness to rule out spinal cord metastasis.

Patient Data

Age: 50 years
Gender: Male
mri

MRI whole spine demonstrates multiple innumerable variable size extramedullary intradural nodules of low SI on both T1 and T2 WI. Those nodules are enhancing on T1C+ associated with leptomeningeal enhancement resembling "sugar coating".

Associated intramedullary high SI on T2 WI and mild cord expansion at lower cervical and upper thoracic spine representing cord edema is seen. Multiple ring enhancing lesion at the posterior fossa and soft tissue of the right occipit are seen as well. Dilated 4th ventricle.

Case Discussion

The findings are in keeping with the clinical suspicion of spinal cord metastasis.
