Spinal schwannoma

Diagnosis probable

Presentation

Chronic low back pain radiating to the right lower limb.

Patient Data

Age: 70 years
Gender: Female
mri

Well-defined mass measuring 26 x 18 x 16 mm, widening the right L5-S1 exit foramen of low signal on T1, high signal on T2 with vivid enhancement except for a small area of cystic change.  Associated adjacent vertebral bony scalloping.

Degenerative changes of the lumbar spine with no disc herniation.

Case Discussion

The MRI features are suggestive of a spinal schwannoma along the right L5 nerve root.

Schwannomas are usually indistinguishable from neurofibromas, however, schwannomas are frequently associated with hemorrhage, intrinsic vascular changes, cyst formation and fatty degeneration. These findings are rare in neurofibromas.
