Splenic and peritoneal hydatidosis

Diagnosis almost certain

Presentation

Chronic abdominal pain.

Patient Data

Age: 50 years
Gender: Female

Very large cyst distorting the splenic contour with peripheral rind of enhancement measuring up to 13 cm, with numerous smaller cysts containing daughter cysts of variable amounts of peripheral calcifications extending inferiorly. Separate cyst with floating membrane in the mid abdomen. Additional cystic lesions in the left adnexa which appears to be separate from the ovary and containing likely peripheral daughter cysts, and a larger cyst measuring 7 cm in the pouch of Douglas. Small amount of ascites.

Case Discussion

Large burden of splenic hydatid disease with some peritoneal disease (due to rupture). Notice the different phases of disease with the lower splenic cysts containing the "classic" daughter cysts and the mid abdominal cyst with floating membrane.
