MRI demonstrates prominent dural thickening and enhancement throughout all compartments. The sagittal midline image confirms general inferior drooping of the posterior fossa contents and splenium of the corpus callosum.
Findings are consistent with intracranial hypotension.
Case Discussion
Clinically this patient described the headaches as postural (worse on standing) and went on to have a lumbar puncture which confirmed low CSF opening pressure establishing the diagnosis of spontaneous intracranial hypotension.
A CSF leak or CSF-venous fistula is most likely found in the spine. Followup or location of CSF egress is not available.
