No fracture, periosteal reaction or malalignment.

Asymmetric increased uptake within the lateral midfoot on all three phases.

Stress fracture of the fourth metatarsal shaft with periosteal reaction and cortical lucency. 

Case Discussion

This case demonstrates the low sensitivity for early-stage stress reactions/injuries and further imaging is warranted in cases of persisting symptoms.
