Striated nephrograms in pyelonephritis

Presentation

Lower urinary tract symptoms and flank pain. E. coli urinary tract infection (UTI) diagnosed on urinalysis. Not improvement despite 5 days of oral antibiotics.

Patient Data

Age: 20 years
Gender: Female

Extensive wedged-shaped areas of hypoattenuation in both kidneys, which appear edematous. Minimal perinephric fat tranding bilaterally. Small amount of pelvic free fluid, possibly physiological. No renal or ureteric calculus.

Case Discussion

A good example of bilateral striated nephrograms. The appearance describes the alternating bands/wedges of high and low attenuation seen on contrast-enhanced CT.
