Presentation
Presenting with imbalance and motor complaints for the last 3 years. The patient was apparently well until about 6 years of age. On inquiry, there is a history of measles at 3 years of age.
Patient Data
There are confluent T2, FLAIR hyperintensities in bilateral periventricular and subcortical white matter, corpus callosum, which appear hypointense on T1, and do not show restricted diffusion or blooming on gradient. Mild volume loss with associated ex-vacuo dilatation of the left lateral ventricle is seen.
Case Discussion
There are confluent areas of altered signal intensity in bilateral periventricular and subcortical white matter as described with associated mild volume loss.
These are non-specific findings and may be seen in certain metabolic disorders and leucoencephalopathies, and require correlation with clinical and laboratory findings.
In the given clinical scenario, the possibility of subacute sclerosing panencephalitis seems likely.
