Subconjunctival fat prolapse

Presentation

Suddenly developed subconjunctival fatty mass on the left. Repeated wheezing before the incident.

Patient Data

Age: 70 years
Gender: Male

In the left supero-lateral aspect of the orbit there is a 14 x 10 x 3 mm elongated soft tissue mass, which is continuous with the intraconal fat, and shows identical signal on all sequences. 

A more subtle, thin, about 13 mm long similar mass is seen in an identical position adjacent to the right globe. 

Partially depicted small T1 hyperintense focus in the fourth ventricle, which could represent a small epidermoid cyst.

Case Discussion

Bilateral (although only the left one was clinically significant) subconjunctival fat prolapse, likely due to the repeated sudden increase in intraorbital pressure combined with age-related laxity of the connective tissue. No underlying more sinister cause was observed.
