Case contributed by Dalia Ibrahim
Diagnosis certain

Presentation

Epigastric pain. Incidental finding on CT scan of the abdomen and pelvis.

Patient Data

Age: 40 years
Gender: Male
ct

Subhepatic position of the cecum.

Few mildly enlarged (subcentimeter) mesenteric lymph nodes.

Mural thickening of the gastric body and antrum as well as the proximal duodenum.

Case Discussion

The subhepatic position of the cecum and appendix is a normal variant which might present a diagnostic dilemma in cases of acute appendicitis because tenderness in such cases is not typically located at McBurney's point.
