Subscapularis calcific tendinosis

Presentation

A young male developed acute onset of the right shoulder pain for the last two days. There was associated severe restriction of the shoulder joint movements. There was no history of trauma.

Patient Data

Age: 30 years
Gender: Male
ultrasound

There is a large calcification ( 28 x 14 x 6 mm ) involving the subscapularis tendon. The tendon itself is intact. The rest of the rotator cuff tendons are normal. There is also mild bursal effusion.

There is a soft tissue calcification anterior to the lesser tuberosity of the humeral head. 
There is no fracture/ dislocation/ bone lesion.

Case Discussion

A young male presented with acute onset, the non-traumatic origin of the shoulder pain, and restricted movements. Ultrasound and radiographs show subscapularis calcific tendinosis. A leak of some amount of calcium is the cause of the acute onset of severe pain and reactive mild bursal effusion.
