Supraspinatus tear

Diagnosis almost certain

Presentation

Left shoulder trauma four days ago.

Patient Data

Age: 75 years
Gender: Female
ultrasound

Echogenic fluid is seen in subacromial - subdeltoid - subchoracoid bursa as well as in posterior gleno-humeral joint space and biceps tendon sheath.

Significant thinning of supraspinatous tendon (2 mm) suggesting full thickness tear. Retraction of supraspinatous tendon is 2 cm.

Infraspinatous tendon is intact.

Mild fatty infiltration of suprapinaous and infraspinatous muscles is seen. 

Biceps tendon, subscapularis tendon - normal.

Modetate irregularity of the greater tuberosity is seen.

IMPRESSION :

  • Full thickness, anterior, degenerative tear of supraspinatous tendon with retraction of 2 cm.
  • Glenohumeral joint hemarthrosis.

SURGERY: Open cuff repair performed. Findings confirmed.
