Left subdural hematoma with hypodensities interspersed within the hyperdense collection (swirl sign).
Case Discussion
The swirl sign refers to the noncontrast CT appearance of acute extravasation of blood into a hematoma. It represents unclotted fresh blood which is hypoattenuating than clotted blood which surrounds it.
