Talar dislocation with significant lateral displacement of the talus and ankle.  No definite plain radiographic evidence of a fracture.

Case Discussion

Talar dislocations are secondary to high energy trauma to an inverted,plantar flex foot; often the result of significant falls or car accidents. The prognosis of a talar dislocation is poor, with avascular necrosis and osteoarthritis common after repair due to the bones limited blood supply 1,2

A recent systematic review of talar dislocations found that 97% of cases were open injuries 1.
