Presentation

Motor vehicle collision. Tender and swollen left ankle.

Age: 60 years
Gender: Female
x-ray

Comminuted talar fracture with subluxation. 

Left ankle

There has been interval incomplete reduction of the comminuted talar fracture. There is intra-articular (ankle and subtalar joint) extension and a wedge shaped fragment lying within the talocalcaneal joint space. The posterior talar fragment remains dorsally displaced and the talonavicular joint is anteriorly subluxed.

x-ray

There has been open reduction internal fixation of the talus. There has been no significant interval change.

Talar head is at risk of AVN if there is a fracture through the talar neck.
