Presentation

Chronic low back pain radiating to buttocks and mild right sciatica.

Patient Data

Age: 30 years
Gender: Male
mri

Voluminous perineural cyst occupying the spinal canal from S2 to S4 (slightly inclined to the right) with diameters of 7 x 2.3 cm along CC and AP diameters respectively, with completely fluid equivalent signal and associated bony remodeling of the sacrum secondary to the mass effect. The cauda equina is normal.

Case Discussion

Tarlov cysts are benign perineural cysts, typically occurring in the lower lumbar and sacral spine, usually seen at the neural foramina arising from the exiting nerve roots. These benign cysts are usually asymptomatic, however, sometimes they can grow to a large size and cause pressure symptoms and bone remodeling as in this patient.
