Presentation

Elderly male with a history of heart failure and left ventricular hypertrophy.

Patient Data

Age: 70 years
Gender: Male
Nuclear medicine

Findings negative for cardiac amyloidosis by nuclear medicine imaging criteria.

Thoracic scan in the anterior view performed 1 hour after administration of 99m-technetium pyrophosphate. 

Semi-quantitative cardiac Perugini grading scale is 0.

There is no intense radiotracer accumulation in the myocardium. The region of interest markers placed over the myocardium and contralateral right lung for quantification of radiotracer activity. The total counts within the myocardial region of interest divided by the contralateral counts were used to calculate the heart-to-contralateral ratio.

A ratio is >1.5 is abnormal. Here, the ratio <1.5 (normal).

Case Discussion

If clinically necessary, a cardiac MRI can be performed to evaluate for left ventricular hypertrophy.
