Tension hydropneumothorax

Presentation

Sudden onset breathlessness.

Patient Data

Age: 35 years
Gender: Female
x-ray

Large right hydropneumothorax is present causing complete collapse of right lung with contralateral tracheomediastinal shift.

Immediate post intercostal tube drainage radiograph shows some expansion of collapsed right lung with centralization of trachea and mediastinum.

Case Discussion

Straw colored fluid was obtained on drainage with high adenosine deaminase level. Patient was started on antitubercular therapy.
